Health by Design, a patient centered, health and wellness group in San Antonio, TX has an immediate full-time opportunity for a Licensed Professional Counselor (LPC). This individual will be a key member of our Behavioral Health Collaborative Care Team, embedded in a Primary Care setting, working closely with the entire Primary Care Physician Team.
Will support and coordinate the mental health care of retired San Antonio Police and Fire Fighters and their dependents, including providing crisis intervention. Will counsel patients who are experiencing depression, anxiety, addictions, substance abuse, PTSD, family, parenting, marital problems, stress management, self-esteem, and aging; to promote optimum mental and emotional health. This is a patient population that is extremely rewarding to care for and appreciative; not to mention responsible when it comes to keeping their appointments!
With a mix of in-person and virtual care, this individual will see patients between a total of four clinics until the need arises for each clinic to have its own full-time LPC (the program is growing quickly). Clinics are located in Westover Hills (Culebra and Rogers Road), Fair Oaks (IH-10 and Ralph Fair Road), Schertz (IH-35 and Schertz Parkway) and Brooks City Base (South San Antonio). Monday – Friday, 8:00am - 5:00pm, no evenings or late appointments, no weekends, no holidays and no on-call. Prior experience or knowledge of working in a Collaborative Care Model: The Collaborative Care team is led by a primary care provider (PCP) and includes behavioral health care managers, psychiatrists and frequently other mental health professionals all empowered to work at the top of their license. The team implements a measurement-guided care plan based on evidence-based practice guidelines and focuses particular attention on patients not meeting their clinical goals (see American Psychiatric Association, www.psychiatry.org for more detailed information).

Essential Job Functions:
Completes comprehensive screening and assessment within program guidelines and formulates a diagnostic impression by conducting client and /family interviews and identifying strengths, weaknesses, problems, and needs for the development of a treatment plan.
Prepares individualized treatment plan, in accordance with established standards and deadlines, consistent with assessment and in conjunction with clients, to include goals, intervention, necessary support or referrals.
Coordinates care with the patient’s medical provider and other mental health providers through weekly scheduled caseload consultations.
Consultant monthly with the Psychiatric, and Collaborative Care team to include meetings with the PCP. Communicate treatment recommendations to the patient’s medical provider (PCP) where necessary.
Conducts individual counseling sessions using evidence-based treatment in accordance with treatment plan, or as necessary for crisis intervention; provides clients with feedback and support or encouragement to address behaviors and attitudes, or family, social or personal problems.
Provides patient education regarding common mental health and substance use disorders and available treatment options to enable patients to understand and accept responsibility for the recovery process and acquire necessary coping and behavior management skills.
Systematically track treatment response and monitor patients for changes in clinical symptoms, treatment side effects and complications
Consults with medical provider and the Psychiatric Consultant to facilitate treatment plan changes for patients who are not improving as expected, and who may need more intensive or more specialized mental health care.
Tracks patients follow up and clinical outcomes using a registry.
Documents in the registry in-person and telephone encounters to identify and re-engage patients.
Maintains documentation in accordance with HIPAA, organizational regulatory accreditations, and contractual standards. This includes but is not limited to treatment plans, narrative progress notes, discharge summaries, treatment plan.
Review and maintain client’s records and charts in accordance with HIPPA, contractual standards, organizational accreditations.
Maintains regular communication verbally and in written form with insurance providers and others that are authorized to provide continuity of care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services, housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develops client discharge and relapse prevention plans which integrate aftercare treatment and the use of appropriate referral resources.
Essential Job Requirements:
Education: Master’s degree in counseling or clinical psychology or a related field of study
Experience: Completion of degree and all post-degree supervision for licensure and at least two years of post-degree experience
Certificates, Licenses, Registrations: Must be currently licensed in the state of Texas as a Licensed Professional Counselor (LPC).
Must stay up to date and current on requirements to practice with license in the state of Texas.
Required Skills:
Thorough understanding of the 12-Step and Relapse models
Knowledge of Social Learning Theory, Cognitive Restructuring, Behavioral Therapy models and/or Therapeutic Community Model and working with different counseling styles
Experience in assessment and treatment of PTSD including CBT and exposure therapy.
Working understanding of the unique needs/dynamics of the client population
Possess excellent writing skills.
Willingness to work with a collaborative care team approach and to learn new skills pertaining to this treatment model.
Ability to communicate effectively with treatment staff.
Ability to collaborate with physicians and staff on individualized patient action plans.
Understanding of the requirements of supervision, including meeting individual and program goals
Excellent organizational skills to track and coordinate collaborative care.
Must be trained in Cardiopulmonary Resuscitation (CPR) and First Aid
Desired skills:
Prior experience of delivery of mental health treatment in the Primary Care setting or the Collaborative Care Model
Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice.
Experience in Crisis intervention and/or Emergency psychiatry
Experience in delivery of trauma-informed care
Experience with elderly population
