About the Role
We’re seeking an experienced, compassionate Registered Nurse who brings both clinical excellence and a passion for helping patients live healthier, more purposeful lives—especially those managing diabetes.
This newly created position will work directly alongside our Diabetes & Lifestyle Medicine Physician and a multidisciplinary team that includes our Clinical Pharmacist and Care Navigators. Together, you’ll provide education, guidance, and support to patients and their families, with a strong focus on diabetes care and lifestyle medicine.
A Certified Diabetes Educator (CDCES) designation is preferred but not required—we’ll support the right candidate in achieving certification within their first year of employment.
You’ll rotate among four of our San Antonio Health & Wellness Centers—serving retired First Responders and their families—typically working at one site per day (Westover Hills, Fair Oaks, Schertz, or Roosevelt).
This role offers exceptional work-life balance:

Key Responsibilities
Provide therapeutic, patient- and family-centered nursing care in alignment with physician orders and organizational policies
Lead and deliver diabetes and lifestyle education to empower patients toward improved self-management
Collaborate with physicians, pharmacists, and care team members to ensure integrated, value-based care
Assist with procedures, medication administration, and monitoring (including phlebotomy, blood pressure, blood sugar, EKGs, and other vital measures)
Support patients with scheduling, test follow-ups, medication refills, and referrals
Participate in quality improvement initiatives and ongoing professional development
Serve as a clinical and educational resource for both patients and team members
Demonstrate flexibility by providing coverage and clinical support across our San Antonio clinics
Ideal Candidate
Seasoned Registered Nurse with at least 5-7 years of clinical experience
Strong interest—or prior experience—in diabetes education and management
Compassionate communicator who builds trust and encourages lifestyle change
Thrives in a collaborative, purpose-driven environment
CDCES certification (or willingness to obtain within the first year, with organizational support)
Registered Nurse (RN) license in Texas (required)
Basic Life Support (BLS) certification (required)
Associate’s Degree, Hospital Diploma, or Bachelor’s Degree in Nursing (preferred)
Minimum of 5-7 years direct patient care experience
Prior Endocrinology/Diabetes experience highly preferred
