About The RoleThe behavioral health care manager is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental health care of patients on an assigned patient caseload with the patient’s medical provider (Primary Care Physician) and, when appropriate, other health providers supporting programs such as diabetes prevention and lifestyle management. The role will involve coordination of care, case management, support, and counseling to those eligible patients experiencing difficulties including depression, chronic pain, anxiety, addictions, substance abuse, ADHD, PTSD, insomnia, interpersonal problems, stress management, self-esteem, and aging; to promote optimum mental and emotional health. The behavioral care manager will be familiar with or be willing to learn and promote lifestyle approaches such as nutrition and exercise.In this role, you will have the chance to work with a care model that has proven results and to connect with patients and team members in a holistic, dynamic, tech-forward way. You will also be able to contribute to the ongoing development and expansion of the program. Essential Job Functions
  • Support the mental and physical health care of patients on an assigned patient caseload. Closely. coordinate care with the patient’s medical provider, psychiatric consultant and, when appropriate, other mental health providers.
  • Comprehensively screen and assess patients for common mental health and substance abuse disorders to formulate a diagnostic impression including level of impairment and needs for a treatment plan
  • Prepare an individualized treatment plan, in accordance with established standards and deadlines, consistent with assessment and in conjunction with patients, to include goals, interventions, necessary support, or referrals. Facilitate patient engagement with the plan and follow-up care.
  • Provide patient education about common mental health conditions, substance abuse disorders, lifestyle changes, and the available treatment options.
  • Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications using validated scales such as PHQ9 and GAD7.
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Provide brief therapeutic interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments or other clinically indicated services outside of the organization (e.g., social services such as housing assistance, mental health specialty care, substance abuse treatment).
  • Participate in regularly scheduled (usually weekly) caseload consultations with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
  • Track patient follow-up and clinical outcomes using a registry.
  • Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultants, and other treating providers.
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
  • Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.

Essential Job Requirements
  • Education: Master’s Degree in Counseling or Clinical Psychology or a related field of study
  • Experience: Completions of degree and all post-degree supervision for licensure and at least 5 years experience as a certified or licensed professional counselor
  • Certificates, Licenses, Registrations: Must be currently licensed in at least one of the states listed KY, TN, FL, TX, IN, GA, AL, MD, OH, MS, VA, NC, SC, OK or LA as a Licensed Professional Counselor or LCSW

Required Skills
  • Ability to navigate an EHR and other technology including communication platforms, a registry, and databases
  • Willingness to work with a collaborative care team approach and to learn new skills pertaining to this treatment model including additional required training
  • Ability to communicate effectively with treatment staff
  • Ability to collaborate with physicians and staff on individualized patient action plans
  • Understanding of the requirements of supervision, including meeting individual and program goals
  • Excellent organizational skills to track and coordinate collaborative care
  • Experience with screening, assessment, and treatment of common mental health and/or substance abuse disorders
  • Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders
  • Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
  • Experience with evidence-based counseling techniques
  • Willingness to obtain multiple state licenses expensed by Medici

Desired skills
  • Prior experience of the delivery of mental health treatment in the Primary Care setting or the Collaborative Care Model
  • Basic knowledge of psychopharmacology for common mental health disorders that is within the appropriate scope of practice
  • Experience in Crisis intervention
  • Experience in the delivery of trauma-informed care
